Celebrate Downtown Trunk & Treat festival registration form

All organizations / individuals that participate are asked to do the following:

  1. Have a decorated vehicle and staff member at the event the entire 5:00-8:00pm time period
  2. Supply participants with trick or treat candy (all candy must be individually wrapped)
  3. Participate in the judging of the vehicle decoration contest.
  4. All registered vehicles must be setup in their assigned space # by 4:30pm 1 space per vehicle
  5. Festival time is 5:00-8:00pm (must be present entire time)6.Must follow family friendly rules: appropriate theme, noise levels, edited music, no alcohol, no smoking
  6. Meet city guidelines for insurance (vehicle) ect..
  7. Vehicle participation is FREE of charge.
  8. Spaces are limited and will be made on a first come first served format.
  9. One vehicle per registration form!!!
  10. Deadline for submission: October 20, 2017 at 5pm to the Mattoon Chamber of Commerce via drop off, email matchamber@consolidated.net, or Fax (217) 235-5786
  11. You will be notified by email of space # for day of event
